Request for Application

The Request for Application is built around the Affiliate’s Statement of Need which conveys the guidelines for the types of programs Komen and the Affiliate are willing to fund. The Statement of Need and RFA are both based on findings from the bi-annual Community Profile.

The RFA also sets the rules for applying for Komen Louisville Affiliate grants, including deliverables under the grant contract, evaluation criteria, grants structure and budget and other qualifying guidelines that must be met before the proposal is sent to review.

Applications for Community Grants in the amount of $5,000 to $75,000 will be accepted for breast health or breast cancer screening, treatment and education for the fiscal year April 1, 2012 to March 31, 2013. 

Priority will be given to projects targeting the under-insured and uninsured populations.

Programs must reach the Komen Louisville Affiliate Service Area.  The Louisville Affiliate services:  Breckinridge, Bullitt, Grayson, Hardin, Jefferson, Larue, Meade, Nelson, Oldham, Shelby, and Spencer counties in Kentucky and Clark, Floyd, Harrison, and Scott counties in Indiana.
